Genentech Patient Foundation
The Genentech Patient Foundation provides free Genentech medicine to
people who don’t have insurance coverage or who have financial
concerns and people who meet certain income criteria.*
*To be eligible for free Genentech medicine from the Genentech Patient Foundation, insured patients who have coverage for their medicine must have exhausted all other forms of patient assistance (including the Genentech BioOncology Co-Pay Card and support from independent co-pay assistance foundations) and must meet financial criteria. Uninsured patients and insured patients without coverage for their medicine must meet difference financial criteria.
The Genentech BioOncology® Co-Pay Assistance Program
Genentech offers the Genentech BioOncology Co-pay Assistance Program to help eligible patients with the out-of-pocket costs associated with their Herceptin prescription.
Some health plans might not accept a co-pay assistance program. Patients should contact their insurance providers to find out if their plan allows the use of co-pay assistance program.
The Genentech BioOncology Co-pay Assistance Program is available only for commercially insured patients, except for patients who are covered by Medicare Part A for hospital services and have commercial insurance for their outpatient medical and pharmacy benefits. It requires a valid, on-label prescription and cannot be combined with any other rebate/coupon, free trial or similar offer for the specified prescription. It is available only for a patient (or their guardian) who is 18 years or older. It is not valid for medications the patient receives for free or that are eligible to be reimbursed by private insurance plans or other health care or pharmaceutical assistance programs that reimburse the patient in whole or in part for the medication. It is valid only for Genentech products in the United States and U.S. Territories. The Genentech BioOncology Co-pay Assistance Program is not health insurance or a benefit plan. Please see full terms and conditions at https://www.copayassistancenow.com/#/terms-conditions.

†Insurer/payer policies are subject to change. The completion and submission of coverage or reimbursement-related documentation are the responsibility of the patient and the healthcare provider. Genentech makes no guarantee concerning coverage or reimbursement for any service or item. Inclusion of a plan or product is not intended to imply a recommendation of a particular plan or product.
